Joseph (Joe) is an in-network provider with multiple insurance companies, including Medical Assistance.  For general inquiries regarding coverage for services, you are welcome to call Joe directly, at Transformation Counseling.  For more specific questions regarding your individual, please call the number provided on the back of your insurance card.

If not able to use your insurance, services may also be provided using self-pay or at out-of-network.  If using insurance, your insurance company will be billed electronically, through Joe’s billing agent, Great Lakes Medical Billing Company, of Nisswa, MN.  At the time of service, fees such as your co-pay, co-insurance, amount toward deductible, and/or other fees may be paid by using cash, check, Master Card, Visa, Discover, or American Express.  Finally, for those those clients without insurance, or opting not to use, a Self-Pay option is also available.  Self-Pay rates for 45-50 minutes sessions are 100.00. However, depending on income, a sliding fee scale is available for those in financial need.  Funds from a Health Savings Account (HSA), or Flexible Savings Accounts may also be used to help clients cover costs for services.  Please feel free to call Joe or Transformation Counseling with questions about payments and/or fees.
