Welcome to Transformation Counseling.  If able, please download and complete the following packet of Intake Forms, as they apply (adult or minor), prior to your first visit.  Specifically, one packet is for you to complete, while the other forms are for you to keep and contain important information regarding rights as a client for privacy, expectations for services, and confidentiality regarding your health information.

A Release of Information form has been provided, if you wish for me to contact your medical provider or another provider.  If you have any questions regarding the forms, please just note those and I will go over these with you at the beginning of your first visit to help clarify.
